Key Responsibilities:
- Provide integrated services that are recovery-focused, culturally competent, and patient-centered.
- Collaborate within a multidisciplinary team to support health behavior change.
- Conduct comprehensive assessments and develop tailored care management plans.
- Advocate for clients\' needs including healthcare benefits, housing, education, and employment.
- Coordinate care across various treatment settings including office and community-based settings and provide crisis management when needed.
