OGDEN WEBER COMMUNITY ACTION PARTNERSHIP Circles Resource Case Manager in OGDEN, Utah

Circles Resource Case Manager:

Your natural ability to make connections with people, and to motivate and inspire them to achieve positive results will serve you well as our new Circles Resource Case Manager. You will have a significant impact on helping families progress in their established goals by performing ongoing case management and referral services. Each day will be full of opportunities to interact with others as you develop strong relationships with our Circles program participants and external partnerships. We are Ogden Weber Community Action Partnership (OWCAP), and we are searching for a Circles Resource Case Manager to ensure our program participants are connected to the appropriate community resources.

What you'll need to be our Circles Resource Case Manager: 

  • A multi-tasking, socially poised and motivating team builder         
  • A problem solver with a positive response to pressure      
  • An innovative, "outside of the box" thinker
  • Bachelor's degree in related field required (Social Work, Child and Family, Psychology, etc.)
  • 3+ year's work experience in a related field
  • Experience working with low-income families and community services preferred
  • Bi-lingual preferred
  • Additional education may be substituted for experience (paid or unpaid) or a combination of both is applicable to meet requirements to be hired

What you'll do as the Circles Resource Case Manager:

-         Perform on-going case management and referral services to Circles Leaders and families using a holistic, comprehensive approach. 

-         Work creatively with Circles participants to develop family goals and strategies based on each families' particular circumstances, beliefs, and behaviors.

-         Work with Community Support Manager and Circles Coordinator to meet all requirements, and guidelines. Work as a team with Circles Coordinator, Coaches, and Outreach Workers.

-         Develop relationships with Circle Leaders, Allies, and community volunteers to connect participants to existing community resources.

-         Collaborate with coaches, allies, and other volunteers to assure families' progress.

-         Develop partnership agreements and collaborative networks with community resources and services.
