Perform initial assessments with new or potential clients Conduct in home supervisory home visits every 60 days and annual service plan reviews on a yearly basis Report observed changes in the Client's condition Documents the record whether caregiver services are being delivered in an appropriate manner and are consistent with policies and procedures Document all occurrences per company policy Follow all company policies and procedures * Must have a valid R.N. license in the state of New Jersey*
