Position Overview:
Competently assesses, coordinates and evaluates the spiritual needs of patients and families and provides direct spiritual care in coordination with the patient care team. Develops, documents and delivers a spiritual care plan. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Adheres to hospital polices, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience. Provides worship services, memorials, and support groups for staff as requested.
Job Description :
SPIRITUAL CARE SERVICES.
• Establishes and periodically revises plan of care by comprehensive and ongoing spiritual assessment of patient/families concerns, strengths and limitations.
• Gathers and incorporates data reflective of the patient/families developmental stages and age specific needs into assessment and treatment planning.
• Communicates changes in plan of care to other members of the interdisciplinary team and community resources as appropriate.
• Demonstrates knowledge and understanding of care of the patient/family and provides appropriate spiritual counseling based on the plan of care.
• Applies principles and techniques of spiritual care to increase coping effectiveness of patients/families, interdisciplinary team and community members.
• Provides assistance in planning and/or conducting funerals, memorial services and or other sacramental needs.
• Educates about community resources and provides a link to these services.
• Acts as a resource to staff to assist with spiritual needs and concerns.
• Mentors and fosters a constructive teaching environment that helps Chaplain Students and new Chaplains build confidence in their skills, knowledge and abilities.
DOCUMENTATION.
• Prepares and completes accurate documentation related to patient in medical and departmental records as required.
• Thoroughly reviews charts and makes any necessary changes.
COMPLIANCE.
• Maintains strictest confidence of all patient protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.
• Performs quality spiritual care in compliance with department standards, policies, and procedures, as well as local, state and federal regulatory agencies to deliver the highest level of patient privacy and care.
• Retains certification and completes mandated continuing education requirements to meet facility and regulatory agencies accreditation and licensure requirements.
EDUCATION:
Master's: Divinity
OR Other: Graduate level theological degree from a CHEA accredited organization
OR Other: Equivalent granted by the Association of Professional Chaplains
4 units of ACPE training
CERTIFICATION & LICENSURE:
Board certification through APC, NACC, NAJC, or CASC within three years
SKILLS AND KNOWLEDGE:
Knowledge of death, dying, grief, and bereavement processes.
Knowledge of diverse religious practices and community resources.
Possess interpersonal skills, both written and verbal, to explain sensitive information clearly and professionally to diverse audiences.
Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures to achieve objectives and meet deadlines.
General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook).

Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.

Pay Range is $46.67 to $60.66 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
