Home Care Coordinator for Loyola
Location: Loyola Hospital in Maywood
Loyola Medicine Home Care is a certified home health agency affiliated with Loyola Hospital and a ministry of Trinity Health , one of the nation's largest Catholic providers of home health services, with 1.7 million annual visits. Position Summary
The Home Care Liaison plays a key role in optimizing transitions between home and hospice care. Responsibilities include facilitating referrals to post-acute home care and hospice, educating acute care staff—including physicians—on available services, and supporting business development initiatives related to program expansion.
Qualifications
Strong communication, customer service, and critical thinking skills
Knowledge of Medicare regulations and managed care environments
Proficiency in multi-tasking across multiple computer systems
Active Illinois registration or licensure preferred
Preferred experience:
3–4 years in acute clinical practice
1–2 years in home care direct patient care
1–2 years in intake or sales (a plus)
Valid driver’s license and reliable transportation

Compensation & Benefits
Pay Range: $28.56–$45.70 per hour
