Residential Assistant - Life Education Advancement Program (LEAP)- $18.00/Hr

Closing Date: 12/09/2024

PT 16 Hr/wk

Provides 24/7 support to young adults in a residential setting while teaching daily living skills in accordance with agency values and procedures. Responsibilities include goal planning and progress reporting, and activities promoting skill development required to enhance and maintain healthy living (examples include making and keeping important appointments with healthcare providers, menu planning, laundry, grocery shopping, social skill development, and providing support for ADL). Documentation of services in the Electronic Medical Record System (EMRS). CNA/HHA/PCA/PCT preferred.

Sat, 12:00 pm - 8:00 pm, Sun, 6:45 am -2:45 pm

Duties and Responsibilities:

• Implements designated goal activities in coordination with the Service Coordinator or Director/Coordinator
• Provide crises intervention supports
• Conducts light housekeeping, disinfecting, yard maintenance and snow removal on sidewalks and emergency exits
• Provides emotional support to program participants
• Monitors program participants physical and mental health and report relevant information to the Service coordinator or Director/Coordinator
• Documents interaction in the programs communication log
• Monitors and reports program participants compliance to their conditional release order when applicable
• Provides coverage as needed at any of the Agency sites per Agency need
• Engages in continuous concerted efforts to access, obtain, respond to pertinent Agency information/communication, and disseminate as appropriate
• Provides working conditions that maximize safety for staff and program participants
• Maintain CPR/FA certification
• Other responsibilities of Reliance Health Staff, such as participation in Agency events, adhering to Agency policies and procedures, and training requirements, etc.
• Performs duties while demonstrating Agency core competencies and values

Education and Experience:

• High School Diploma or equivalent
• Work experience in the provision of mental health services preferred
• Valid CT Drivers License, vehicle registration and insurance
• Good driving record
