What will you do?

Clinical Assessment and Care:

  • Conduct suicide and risk assessments
  • Facilitate initial care assessments to determine treatment priorities
  • Create and manage Individualized Service Plans (ISPs)
  • Deliver Skills for Psychological Recovery (SPR) curriculum
  • Focus on skill-building components including problem-solving, positive activities, managing reactions, helpful thinking, and social connections

Care Coordination:

  • Coordinate referrals and facilitate warm handoffs to VA facilities
  • Ensure seamless transition and continuity of care
  • Build relationships with Region 3 service providers
  • Develop and maintain community partnerships
  • Travel 25-50% of time throughout service area

Community Engagement:

  • Attend local veteran events across Region 3
  • Represent Easterseals and Veterans Rally Point in the community
  • Participate in outreach activities
  • Maintain highest ethical standards in all interactions
  • Support special projects as needed

Skills and requirements

  • Bachelor's Degree in Social Work or related field required; Master's Degree preferred
  • License eligible candidates highly encouraged to apply
  • Military service preferred
  • Minimum 2 years direct clinical experience
  • Experience in Crisis Intervention and Suicide Prevention programming
  • Eligibility to become a Peer Support Specialist
  • Strong background in case management and care coordination
  • Experience with community outreach and relationship building
  • Excellent project management and organizational abilities
  • Strong conflict management and trauma-informed de-escalation skills
  • Proficiency with Microsoft Office Suite
  • Self-motivated with ability to work independently and collaboratively
  • Strong attention to detail while managing multiple priorities
  • Valid driver's license with proof of insurance; willingness to use personal vehicle
  • Flexibility to work early mornings, evenings, and weekends as needed
  • Commitment to maintaining confidentiality and professionalism
  • Passion for Easterseals' mission and ability to serve as an organizational ambassador

Responsibilities

  • Conduct thorough assessments of clients' needs and develop personalized care plans.
  • Coordinate services across various sectors including healthcare, social services, and community resources.
  • Provide crisis intervention and management to clients in urgent situations.
  • Monitor client progress and adjust care plans as necessary to ensure effective support.
  • Facilitate communication between clients and service providers to advocate for their needs.
  • Maintain accurate records of client interactio s, progress notes, and service referrals.
  • Supervise support staff or interns when necessary to ensure quality service delivery.
  • Stay informed about relevant policies, regulations, and best practices in social work and case management.

Experience

  • A background in social work or a related field is preferred.
  • Experience with child welfare systems, behavioral health services, or addiction counseling is highly desirable.
  • Familiarity with crisis intervention techniques and crisis management strategies.
  • Knowledge of Medicare processes and Section 8 housing assistance is a plus.
  • Proven ability to work collaboratively with multidisciplinary teams and diverse populations.
  • Strong organizational skills with the ability to manage multiple cases effectively.
