Program Supervisor Parents - Community Health Work Outreach

Northridge Hospital Medical Center is seeking a highly motivated and experienced Program Supervisor to lead our Parents as Teachers (PAT) program. This is a full-time grant-funded positio n, offering a competitive salary and benefits package.Our PAT program provides in-home visits and group activities to families with young children (birth to kindergarten entry), promoting healthy child development and parent-child relationships. We are committed to providing high-quality, evidence-based services that empower families to be their children's first and most important teachers.
Responsibilities
Oversee all aspects of the PAT program, including staff supervision, program implementation, data collection and reporting, and budget management.
Supervising a team of Family Service Workers.
Providing ongoing training, mentoring, and staff support.
Conducting regular staff meetings and performance evaluations.
Ensuring adherence to program guidelines and best practices.
Monitoring program outcomes and making adjustments as needed.
Managing program budget and resources effectively.
Maintaining accurate records and submitting timely reports to the granting agency.
Build and maintain strong relationships with community partners, including schools, healthcare providers, and other relevant organizations.
Participating in community events and outreach activities.
Collaborating with community partners to ensure seamless service delivery.
Representing the PAT program at community meetings and events.
Maintaining accurate grant documentation.
Submitting timely reports to the granting agency.
Participating in grant-related meetings and activities.
Qualifications
Bachelor’s degree in social work, child development, or a related human services
Two years or more of supervisory experience in a social services environment.
Successful experience working with families, children, and/or parents, and knowledge of early childhood education best practices. Knowledge of community-based service programs.
Demonstrated supervisory experience.
Home visiting experience preferred.
Bilingual in English/Spanish preferred.
AHA BLS is required within 90 days of hire.
LA County Fire Card upon hire. (within 90 days of hire.)
Valid CA Driver’s License acceptable driving record and valid car insurance.
Pay Range
$37.81 - $54.83 /hour
