LICENSED PRACTICAL NURSE (LPN) - HOSPICE SERVICES

JOB PURPOSE:

To assist the PruittHealth Hospice team in providing nursing care to hospice patients.

KEY RESPONSIBILITIES:

  • Provides palliative nursing care and ongoing support in the home as ordered on the plan of treatment and nursing plan of care.

  • Functions as a team member demonstrating collaboration with, and responsibility to patient, caregiver, physician, interdisciplinary team.

  • Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Reassesses the patient and notifies the registered nurse (RN) when pain is not manageable on the current medications.

  • Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.

  • Adheres to all agency, payroll, billing, and documentation policies and procedures.

  • Demonstrates knowledge f medications and their correct administration based on age of the patient and patient’s clinical position.

  • Address and supports cultural practices, as long as such practices do not harm others.

  • Completes all documentation accurately and in accordance with company policy and procedures.

  • Makes referrals to interdisciplinary team members and other providers, functioning as a team member and demonstrating collaboration with and responsibility to the patient, physician, and interdisciplinary team.

  • Assists in teaching the patient/family about provisions for care and services.

  • Demonstrates knowledge of medications and their correct administration.

  • Participates in Performance Improvement Program.

  • Participates in and presents inservice programs, team conferences, and staff meetings.

  • Provides skilled nursing care to assigned patients/families as outlined in the plan of care.

  • Participates in the development of the patient/family interdisciplinary care plan.

  • Responds to the physical, mental, social, emotional, and spiritual needs of the terminally ill patient and their families with respect to their age, religious and cultural practices.

  • Makes Nursing Services available on an ongoing basis, including regular nursing visits per the patient’s plan of care and /or patient’s needs and as directed by the RN Case Manager. The LPN may not complete the Initial Nursing Assessment.

  • Monitors educational needs of each patient and provides education, training and support as necessary with appropriate documentation on the nursing visit record.

  • Performs other duties as assigned by the DHS or Administrator.

KNOWLEDGE, SKILLS, ABILITIES:

  • A thorough understanding of Hospice Regulations (federal, state, and local government).

  • Possess excellent communication skills, both oral and written.

  • Excellent organizational skills.

  • Ability to handle confidential matters.

  • Ability to work a flexible schedule.

  • Commitment to the Hospice concept and standards.

  • Ability to interact with a variety of different age groups.

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

  • Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
