Overview
Provides consultation in palliative care, symptom management and supportive care to meet the needs of patients. Informs primary physicians of recommended services and collaborates with other physicians as needed. Facilitates clarification of patient and family plan of care. Implements and creates interventions to advance and improve the palliative care program.
Responsibilities
1.Communicates and collaborates with the Administrative Supervisor and the Director/Clinical Coordinator on clinical and administrative matters.
2.Maintains the confidentiality and priority of all patients and staff information.
3.Utilizes organization and time management skills to direct the operations of the palliative care program. 4.Assures the care delivery of patient care services to improve the standard of care and customer satisfaction for palliative care patients and family.
5.Communicates changes in policy and procedures to Directors/Clinical Coordinators as needed.
6. Refers patients and family members to members of the multidisciplinary team for the continuum of palliative care.
7.Coordinates, facilitates and engages discussions regarding plan of care with patients, family member and medical staff.
8.Delivers care in a manner that preserves and protects patient autonomy, dignity, rights, spiritual beliefs and practices. Serves as a patient advocate.
9. Maintains a therapeutic and professional patient-nurse relationship with appropriate professional role boundaries.
10.Ensures delivery of excellent customer service resulting in positive and improving patient satisfaction scores for the department and Medical Center.
11.Assists in the development of objectives for the Department of Pastoral Care & Education and the Palliative Care Program.
12.Maintains adequate reading materials and resources.
13.Evaluates one's own nursing practice standards and guidelines, relevant statutes, rules and regulations. 14.Plans in collaboration with Director/Clinical Coordinator for the continuing education of the staff.
15.Other duties and/or projects as assigned.
16.Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
1.BSN required.
2.Minimum of five years of clinical nursing experience required.
3.Must be computer literate.
4.Ability to demonstrate a thorough knowledge of the policies and procedures of Nursing and the Medical Center.
5.Must be organized and have time management skills.
6.Ability to demonstrate the ability to objectively analyze situations and take appropriate actions.
7.Ability to foster team building among a culturally diverse staff.
8.Ability to demonstrate decision making skills and problem solving skills.
9.Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 1 hour a day.
10. Requires substantial standing, walking, stooping, and lifting.
Licenses and Certifications Required:
1.License as a registered Nurse of the state of New Jersey.
2.CHPN certification.
