Job Overview:
The Pharmacy Patient Liaison works in the specialty clinics of Newark Beth Israel Medical Center and works closely and collaboratively with the patients, physicians, nurses, dispensing pharmacies, and other healthcare disciplines to ensure that the continuum of patient care as it relates to medication access and adherence is achieved.
Qualifications:
Required:
Registered pharmacy technician in New Jersey. Hospital Pharmacy experience. Certified Pharmacy Technician (CPhT) and knowledge of transplant, infectious disease, and oncology preferred .
'Excellent oral and written communication skills. Excellent phone etiquette skills.
Should be self-motivated and have strong interpersonal skills
Independent worker who is able to multi-task, be detailed oriented, and provide timely service
Experience with Microsoft Office and ability to learn other applications quickly

Essential Functions:
Files and obtains prior authorization from various insurance companies to ensure patients have access to the prescribed medication regimens.
Answer, resolve and triage inbound inquires
Assist patients with the refill of their medications in order to promote and support medication adherence
Coordinates the delivery of patients' medications in order to promote and support medication adherence
Facilitates the access to medications via pharmaceutical patient assistance programs.
Conducts follow-up calls with the patients to ensure medication compliance
Enters prescription information into the database
Discuss with healthcare professionals alternatives to medications prescribed based on information obtained from insurances.
Answer, resolve and triage inbound inquires
Communicate with physicians, nurses and other healthcare disciplines about any issues related to the patient s medication therapy.
Participates in specified departmental and organizational committees as assigned
Documents activities performed in designated format
Recommends improvement and/or participates in process improvement initiatives
Perform other duties as assigned or as necessary
