The Clinical Nurse Navigator offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum
Qualifications:
Required:
ASN or Nursing Diploma from an accredited Nursing Program
Strong communication and organizational skills
Proficient computer skills
Preferred:
BSN
Clinical Navigator experience in Oncology
Certifications and Licenses Required:
Active NJ RN license or compact RN license with NJ endorsement
BLS from the American Heart Association
Oncology or navigation certification required within two years of employment
Scheduling Requirements:
Days
Full Time 37.5 hours per week
Monday Friday, 5 days per week with weekend and major Holidays off
1 day a week option for hybrid after successful completion of probationary period
Essential Functions:
Assess patient needs throughout navigation, matching unmet needs with appropriate services and referrals

Facilitates timely scheduling of appointments, diagnostic testing and procedures to expedite the plan of care and to promote continuity of care.
