Description

TheCoordinator for the Undetectables Programwill be responsible for the provision, direct management, and oversight of all programming related to the Undetectables grant, which includes, but is not limited to client engagement, client enrollment, contract management and outreach services. In addition, this position is responsible for providing direct supervision to the Outreach Worker/Case Manager. The goal of the Undetectables program is to connect clients to comprehensive services which promote U=U.

Essential Job Functions

The following duties are mandatory requirements of the job:

  • Provide direct supervision to the Outreach Worker/Case Manager.

  • Ensure contract deliverables are met, including tracking and provision of the following required services: intake/enrollment, care/service plans, case conferences, treatment adherence counseling in both individual and group settings, modified directly observed therapy (mDOT) sessions, distribution of client incentives, and referrals to needed supportive services.

  • Ensure cross-promotion of Prevention services, including GMHC Testing Center services to clients in need of STI & Hepatitis C testing.

  • Ensure proper documentation of service delivery, incentive distribution, and other documentation, is completed and in compliance with funders and agency requirements.

  • Co-facilitate client engagement and group sessions with other staff as needed.

  • Schedule and lead debriefing of workshops & review of surveys, with appropriate staff, to determine areas of improvement, as needed.

  • Serve as the quality assurance lead, ensuring data quality is high, and that EMR systems data mirror one another and are accurate at all times.

  • Assist in the timely completion of program monthly reports, annual reports, and other reports required.

  • Assist in the development of programmatic policies and procedures.

  • Schedule all activities, including but not limited to virtual and in-person activities, as appropriate.

  • Ensure programmatic activities are compliant with all funder requirements.

  • Manage ordering and inventory of promotional materials to promote programmatic services.

  • Submit monthly board bullets of programmatic activity, as required.

Other Responsibilities

  • Oversee Triad Management for the program including Dashboard (grant performance data collection and tracking) and implement necessary corrective action plans and/or quality improvement plans when needed to meet program deliverables.

  • Promote the Undetectable Program at GMHC and external community events with other department staff.

  • Work closely with the Analytics & Evaluation team to ensure accurate data management, and retrieval for agency wide Continuous Quality Improvement projects.

  • Any other duties as assigned by the supervisor.

Requirements

Education and Certification

  • Bachelor’s degree in human services, psychology, public health, or any other related field preferred.

  • Three to five years of case management experience in client services.

  • The presence of leadership and management skills

Special Skills and Knowledge

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all the following:

  • Deep professional understanding of the needs of the LGBT and HIV community

  • Bilingual in Spanish and English is preferred.

  • Proficiency in Microsoft Office Suite

  • Friendly and personable interpersonal style with excellent communication skills

  • Ability to interface with clients, GMHC staff and external partners.

Salary Description

$70,000
