Description

The Registered Nurse (RN) provides and coordinates the care of patients/families and significant others in the ambulatory care setting by assessing, diagnosing, planning and intervening in actual or potential health problems, including evaluation of the patient’s response to the care. These responsibilities also include patient/family/significant other education and health maintenance strategies. The RN is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals.

The Registered Nurse demonstrates knowledge of the principles of human growth and development over the life span and the ability to assess data reflective of the patient’s status, and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age, specific needs, and to provide the care needed as described by standards and policy and procedures.

RESPONSIBILITIES:

  • Demonstrates the ability to perform clinical/technical/service/administrative tasks.

  • Records, reports and interprets clients’ responses and collaborate with appropriate members of the health team in the Family Practice.

  • Identifies those components of the nursing regimen, which may be delegated to allied nursing personnel and provides the necessary teaching and guidance.

  • Systematically and continuously assesses patient’s physical, psychosocial and spiritual health status and knowledge base to determine patient’s health care and educational needs.

  • Triages all sick and walk-in patients.

  • Establishes therapeutic and supportive relationships with patients and family.

  • Assures proper cost-effective utilization of resources.

  • Maintains an environment that is conducive to meeting patient care needs and that facilitates patient flow.

  • Promotes and provides for patient safety in accordance with state and federal guidelines.

  • Practices and monitors infection control.

  • Monitors patient flow and reassigns staff as needed.

  • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.

  • Participates in the development of other staff members.

  • Meets regulatory, licensure and annual health assessment requirements.

  • Identifies learning strengths and needs.

  • Utilizes learning resources.

  • Attends in-service and continuing education programs.

  • Obtains certification in area of practice.

  • Successful demonstration of competence in area of practice.

  • Demonstrates competence in utilizing the selected information systems.

  • Evaluates the effectiveness of interventions and patient responses and outcomes to care. Collaborates with team members to make necessary changes in plan of care. Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers.

  • Effectiveness in supervising and mentoring LPNs, Medical Assistants, and Nursing students.

  • Understands the necessity of paperwork and fulfills obligations on time.

  • Adheres to and understands nursing policies and procedures.

  • Keeps abreast of new and changing techniques and concepts.

  • Contributes to a cooperative, friendly and mutually supportive work environment. A team player.

  • Ability to assure new assignments.

  • May be required to perform other duties as assigned or when necessary.

    EDUCATION REQUIRED

  • AAS/BS in Nursing

    EXPERIENCES AND/OR SKILLS REQUIRED

  • Minimum of one (1) year experience in an Ambulatory/Medical Surgery/ER setting preferred

  • Bi-lingual, Spanish preferred

  • Demonstrated ability to plan, make decisions, organize, and direct the work of others

  • Minimum of two (2) years nursing experience

  • Demonstrated ability to perform venipuncture and administer medications including immunizations

  • Demonstrated clinical, communication, interpersonal, and organizational skills

  • Demonstrated ability to problem solve using critical thinking

  • Demonstrated basic computer skills including typing

    LICENSES/CERTIFICATIONS REQUIRED

  • Licensed Registered Nurse/Licensed Practical Nurse in New York State

  • Current RN Registration Certificate

  • Current BCLS

  • Current Infection Control Certificate

  • Current Child Abuse Certificate
