Company Overview and Mission Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provision of food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.

Purpose Under the direction of the Director of Emergency Services, the Recovery House and Respite Manager, provides case management to Medical Respite and Recovery House residents.
The manager will coordinate care with the YNHH and VA Hospitals to accept appropriate referrals; networks with Greater New Haven shelter system and mental health/substance abuse/social/medical/housing service systems and collaborates with clinicians and other service providers as well as other agency case managers to empower clients to end their homelessness. The manager also provides short term case management services to adult males leaving a detoxification center and/or waiting for or just leaving an inpatient treatment facility. The manager seeks to create and maintain an atmosphere that reflects the core values of Trauma Informed Services: Safety, Trustworthiness, Choice, Collaboration and Empowerment.
Must serve as the support hub for all aspects related to continuity of care once the client is discharged from the hospital. The Recovery House and Respite Manager provides direct supervision and program management, training, and professional development to all Residential Staff within the Respite and Recovery House Program.
Duties and Responsibilities
- Provide linkages for clients who need social, mental health, educational, vocational, recreational, spiritual, transportation, primary care, visiting nursing and or substance abuse services.
- Assist in scheduling all PCP or Medical Specialty appointments.
- Coordinate transportation to appointments for clients when needed.
- Coordinate and collaborate with other agencies and social service providers.
- Conduct both phone and in -person screening for both Respite and Recovery House.
- Oversee intake, departures and daily roster and programs.
- Enforce house rules, supervise resident chores, and conduct weekly house meetings.
- Attend weekly and monthly agency, program, and network meetings.
- Monitor all cases assigned and complete all documentation/charting required by Columbus House and its relevant funders in support of a 29-client case load.
- Enter and maintain all client charts accurately and timely into the Homeless Management Information System Database Case Worthy
- Hire, train, and supervise up to 14-20 residential support staff, providing all necessary oversight and supervision and ensuring consistency and continuity of services rendered.
- Other related duties as assigned.
Qualifications
- Bachelors degree in human services or related field preferred with a minimum of two years experience working in human services or related fields or five years experience as case manager working in human services or related fields.
- Excellent organizational and time management skills with a deadline-oriented mindset
- Strong written and oral communication skills employing the English language.
- Proficient in Microsoft Office and other common desktop applications.
- Able to use the local network of substance abuse, ment l health, and social services.
- Familiar with criminal justice and able to engage, manage, motivate, and support.
- Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics
- Knowledge base of all aspects of the delivery of person-centered case management
- Able to meet all documentation requirements.
- Knowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, Recovery support, harm reduction, trauma informed care and motivational interviewing.
- Valid drivers license with a good driving history
