JOB TITLE: RN, Director of Patient Services

DUTIES AND RESPONSIBILITIES:

  • Assume responsibilities of the Adult Home/Assisted Living Program in Administrators absence.

  • Responsible for supervision of Home Health Aides in Adult Home/Assisted Living Program.

  • Complete HHA training under the direction of the Administrator.

  • Assist the Administrator in the completion of the UAS-NY assessments, quarterly Supervisory Visits, and resident assessments.

  • Complete 10-day medication training/remediation with HHA’s.

  • Ensure all staff are current with in-service requirements.

  • Provide education to the staff pertinent to their position on a monthly basis.

  • Provide training of the HHA’s with Point Click Care.

  • Rotate on call duties with the Administrator.

  • Supervises Fire and Disaster drills in absence of Administrator.

  • Assist with admission, transfers and discharges, as needed.

  • Complete initial and annual aide evaluations.

  • Assist with follow-up on employee health requirements as needed.

  • Assist with administration of insulin and B12 injections to Adult Home and Assisted Living Program residents as ordered.

  • Responsible for assessing all residents and staff involved in incident reports and ensure forms are completed accurately.

  • Participate in the activities of the Quality Assurance Program, as assigned.

  • Assist the Administrator with employee disciplines.

  • Assist with resident transportation needs, if necessary.

  • Assist the Administrator with the schedule.

  • Monitor resident weights on a weekly/monthly basis and report to physicians.

  • Assume the responsibility of taking the house in the absence of the HHA Supervisor, as needed.

  • Perform personal care aide duties as needed for Adult Home residents and home health aide duties for Assisted Living Program residents, as needed.

  • Administer medication to residents, as needed.

    QUALIFICATIONS:

  • Be 18 years of age, show maturity, emotional and mental stability

  • Shall be able to speak, read and write English, and speak the predominant language of the residents

  • Understand and carry out directions and instructions, record messages and keep simple records

  • Sympathetic attitude towards providing services for residents

  • Have good physical health, as indicated by documentation in the personal file

  • Be required to work up to 8 hours of overtime as staffing changes may occur on a shift to shift basis

    EDUCATION: Registered Nurse with a background in home care and/or care of the elderly preferred. Must possess licensure from the State of New York.
