DEPARTMENT: Occupational Therapy

POSITION: Certified Occupational Therapist Assistant (COTA)

SUPERVISOR: Director of Occupational Therapy

HOURS: Full Time, Monday - Friday

PAY RATE: $18.00-$21.00 DOE

Responsibilities/Duties:

Contributes to the patient/resident evaluation process under the supervision of an occupational therapist. Observes the patient/resident while engaged in activity to collect general data. Interviews patient/resident/family. Uses a structured format determined by occupational therapist and uses structured tests, which do not require interpretation.

Is able to identify patient/resident care related problems, progress, and changes in status and brings them to the attention of the supervising occupational therapist.

Goals entered on treatment plans are carried out as established by the supervising occupational therapist.

Demonstrates knowledge of occupational therapy techniques, modalities, equipment, and instructs patient/resident/family in exercise programs.

Collaborates with patient/resident, families, and supervising occupational therapist prior to patient/resident discharge to formulate an appropriate discharge follow-up plan to include, but not limited to, continuation/discontinuation of occupational therapy services; implementation of home programs; recommend appropriate adaptions/equipment in patient's/resident's home environment; and/or recommends community resources.

Designs, fabricates, and applies orthotic (splint) devices as appropriate under the supervision of an occupational therapist as well as instruct patients/residents and families in the application, care, and precautions of orthotic (splints).

Organizes schedule of patient/resident treatments taking into account the work demands of other employees/departments.

Assists with maintenance occupational therapy programs.

After consulting with occupational therapist, provides objective; accurate; and timely documentation according to policy on assigned patients/residents via progress notes. Update and revise care plans, team conference reports, FIM and MDS sheets, charge/attendance checklist, discharge summaries, and all other miscel-laneous facility forms as necessary. Has supervising occupational therapist co-sign appropriate documentation.

Completes documentation for insurance requests in a timely fashion.

Leaves a written, clear treatment plan (coverage notes) on all assigned patients/residents when scheduled out of the departments. May use phone to address above as appropriate.

If on specific long-term care rotation, writes notes on maintenance residents; and weekly on restorative residents; and obtains co-signature of occupational therapist.

Demonstrates initiative to assist in evaluating and development of current documentation systems, forms, and charting mechanisms to facilitate efficient and effective documentation.

Recommends equipment and/or supplies when inventory demonstrates the need to re-order.

Provides one inservice annually for department/facility staff as well as review current literature review on clinical topics.

Attends one continuing educational programs and/or equivalent annually and upon return, provides inservice to staff.

Participates in program development/revision within the facility. May formulate and design new programs or service to department.

May serve as a clinical instructor in conjunction with occupational therapist to occupational therapist assistant student affiliates following the facility clinical education manual and academic program manual.

Actively participates in own performance appraisal evaluation and sets self goals to accomplish during the following year. Successfully completes annual competency checklist/test.

After consulting with supervising occupational therapist, attends patient/resident care conferences, team conference, and rounds obtaining and reporting patient/resident present level of functioning and any future goals/plans in a clear and concise manner as well as change in condition or treatment of patient/resident.

Engages in timely, creative, and effective use of aides and volunteers. Seeks assistance from occupational therapist as needed.

Maintains open lines of communications with supervisor and occupational therapists to maintain proper and effective sharing of information.

Participates in department QA programs, including collecting of data, analysis, and documentation review.

Assists supervisor in developing new innovative ideas for QA studies.

Accesses medical record on unit for all information applicable to therapeutic intervention. This may include: hospital discharge summary, consults, notes from physician, therapies, social service and case management. Accesses facility nursing notes, care plan, recreational therapy notes, dietary, dental, social service/case management, psych notes, therapy section, consults, physician notes and orders. Also has access to MDS to complete per MDS schedule.

Accesses SOS for the purpose of completing the care plan and MDS per schedule for residents/in-patients. Accesses Microsoft Excel for completion of daily treatment minutes.

Accesses MEDENT to complete evaluation, weekly progress note, re-certification note and discharge as appropriate for treatment plan for their specific discipline. Access to MEDENT to obtain patient appointments as view only, and therapist schedule as view only. Access to MEDENT for outpatient team conference documentation per team conference schedule.

Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws. Adheres to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties.

Performs all other duties as assigned by supervisor to ensure that the department functions and operates as smoothly and efficiently as possible.

Job Requirements:

Qualifications:

Graduate with an Associate of Service degree from an Occupational Therapy Assistant Accredited program.

Basic entry level knowledge gained through normal course of education and student clinicals.

Possess a license of the state of New York to practice as an Occupational Therapist Assistant. Successfully completed the National Certification exam and be credentialed as a COTA, is preferred.

CPR certification.
