Must be licensed through the state of Delaware.

This position will provide individuals, families, and groups with the psychosocial support needed to cope with chronic or acute mental illness. Services include advising and educating family care givers, providing patient education and counseling, and making referrals for other services. Provides care and case management or interventions designed to promote health and address barriers to access to healthcare. Facilitates continuity of care. Must have a LMSW or MSW with plan to be licensed by 2024.

Job Type: Full-time

Pay: $28.05 - $40.00 per hour
