GENERAL RESPONSIBILITIES
To provide nutrition education services to our patients receiving services at our Nampa 1st clinic with an emphasis on our diabetic, pre-diabetic, and hypertensive patients.
ESSENTIAL DUTIES
- Schedule and provide nutrition education and counseling to patients with an emphasis on those patients who have been diagnosed with prediabetes or diabetes.
- Provide comprehensive case management for assigned caseload to include but not limited to, patient education, establishing self-management goals, referral services, and collaboration with clinical, pharmacy, social service, and medical staff.
- Achieve quality clinical outcomes, as evidenced by improvement in pre-determined care indicators. This will be by real-time patient encounters, implementing treatment plans, and collaboration with the full medical team.
- Provide clear written documentation in the patient record of all visits, and treatment plans within 48 hours of the visit.
- Participates in other group meetings, as appropriate.
- Work with our Dietician Internship Program by utilizing interns within the Dietician Program and working with the contracted clinical supervisor of the program.
MINIMUM QUALIFICATIONS
- Current state of Idaho Dietetics licensure.
- Demonstrated experience in nutrition education.
- Strong communication and organizational abilities.
PREFERRED QUALIFICATIONS
- Experience working with a wide variety of patient populations.
- Bi-lingual English/Spanish.
