We are seeking a compassionate and skilled Licensed Social Worker to join our team and make a difference in the lives of individuals needing home-based care.
Job Summary:
The Home Care Licensed Social Worker will provide emotional support, resource coordination, and advocacy for clients and their families. You will work closely with an interdisciplinary team to ensure clients receive personalized care plans that address their physical, emotional, and social needs.
Key Responsibilities:
Conduct psychosocial assessments to evaluate the needs of clients and families.
Develop and implement care plans in collaboration with healthcare professionals.
Provide counseling and emotional support to clients and their families.
Assist clients in accessing community resources, benefits, and services.
Monitor and document client progress, ensuring compliance with care plans.
Collaborate with medical staff, therapists, and caregivers to ensure holistic care.
Educate clients and families on coping strategies and available support systems.
Maintain accurate and timely documentation in accordance with regulatory standards.
Qualifications:
Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of Michigan
Master’s degree in Social Work from an accredited program.
Experience in home care, healthcare, or a related field is preferred.
Strong interpersonal and communication skills.
Knowledge of community resources and healthcare systems.
Ability to work independently and collaboratively in a team environment.
Proficiency in electronic medical records (EMR) systems is a plus.
