Job Description: Star Home Health Agency is seeking dedicated LPNs/RNs to join our team in providing skilled nursing care to patients in the comfort of their own homes. As a Home Health Nurse, you will travel to various locations to deliver compassionate and high-quality healthcare services.
Responsibilities:
- Conduct comprehensive assessments of patients' health status.
- Develop and implement personalized care plans in collaboration with the healthcare team.
- Administer medication, wound care, and other treatments as prescribed.
- Monitor patients' progress and adjust care plans accordingly.
- Educate patients and their families about proper healthcare management and preventive measures.
- Maintain accurate and up-to-date medical records.
- Communicate effectively with physicians, therapists, and other healthcare professionals.
Qualifications:
- Associates Degree in Nursing.
- Current and valid LPN or RN license in the state of Ohio.
- Ability to pass a drug test and criminal background check.
- Previous experience in home health or a similar setting preferred.
- Reliable transportation with a valid Ohio Driver's License and proof of auto insurance.
- Ability to consistently attend scheduled shifts and provide reliable care to patients.
