LPN Nurse

Job Description

The primary function of an LPN is to assist with the care of all patients, regardless of age, during their shift. The LPN is responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. They effectively provide Person Centered care and services.

Responsibilities

• Taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patient's/residents conditions

• Practicing ethically and professionally within their scope; advocating for patients

• Maintaining clinical competence specific to their care area including organization required education within the stated timeframe

• Coordinating care with the interdisciplinary team

• Documenting appropriately and thoroughly in EMR/Point Click Care

• Keeping other nursing personnel and the responsible RN informed regarding changes in the condition of patients/residents and/or other pertinent observations

• Participating in unit councils and staff meetings

Essential Skills

• Graduate of a Nursing Program

• Current NYS LPN License

• CPR/BLS certification

• At least 2 years of experience

Work Environment

2 full-time LPN positions are available with multiple shifts.

The pay range for this position is $25.00 - $30.00
