This position is assigned to the Healthy Start Program and provides case management services, including home visits to pregnant women and infants up to age 3. The incumbent also provides assistance to the contracted county health department OB provider and serves as a liaison between the CHD OB patients, Healthy Start, and WIC. Maintains appropriate certifications in order to provide necessary wrap-around services. This position requires travel.

CHD OB Clinic–Serves as primary contact to OB providers in both Jefferson and Madison Counties. Assists providers in providing care to OB patients

  • New Patients

  • Ensure PEPW paperwork is completed for new patients

  • Maintain and update OB work up packets for new patients

  • All Patients

  • Communicate with front desk with billing/ coding/ scheduling issues

  • Prepare for clinics by reviewing all schedules the day prior

  • Work up clients up for Clinic

  • Obtain Prior authorization for medications per clients’ needs

  • Complete necessary charting in HMS and any paperwork associated with new and existing patients

  • Scan and import medical records to clients’ chart after reviewed and signed off by provider

  • Scan and fax medical records to TMH Family Residency HUB weekly/ TMH Triage as needed

  • Provide follow up on Labs, referrals, and any identified needs

  • Return follow up calls to patients

  • Clinic Operations

  • Serves as lead nurse in OB program.

  • Ensure prenatal client and children’s immunizations are up to date

  • Ensure appropriate OB clinic supplies/ vaccines/ medication are ordered for clinic needs

    Healthy Start IPO (Improve Pregnancy Outcome)- Follow up in assessing prenatal care and other needed services in an effort to help eliminate risk factors associated with preterm delivery, low birth weight and infant mortality.

  • Identify OB clients who have not been funneled through the Healthy Start Coordinated Intake and Referral System and refer accordingly.

  • Recruit and engage CHD OB clients into the Healthy Start program. Act as liaison between the OB clinic and the Healthy Start Program.

  • Make home visits as needed.

  • Provide hands-on education to mom and infants when necessary.

  • Provide breastfeeding, parenting, and childbirth, postpartum, birth control, education one on one or through conducting classes

  • Link OB Clients to WIC and other ancillary services

    Healthy Start Care Coordination - Utilizes the Prenatal and Infant Pathways as outlined in the Healthy Start contract. Assists clients in accessing services. Meets twice monthly with program manager or lead worker to review cases and new documentation.

    Program Documentation- Enters care coordination documentation electronically in a timely manner and ensures appropriate documentation into the Well Family System.

    Maintains appropriate certifications in order to provide necessary wrap-around services. Performs all other duties as assigned.

    Required Knowledge, Skills, and Abilities:

    Able to work independently and as a team: ability to handle multiple tasks at once; must be organized and detail oriented.

    Able to communicate effectively and deal with the public.

    Knowledge of computer software to enhance the productivity of the CHD programs.

    Qualifications:

    Minimum: Licensed Practical Nurse FLORIDA STATUTE REFERENCE: FS 464

    A valid Florida Driver's License is required Computer proficiency.

    Preferred: Case management and/or home visiting experience.

    Spanish Speaking Preferred but not Required
