Overview NorthPoint has been providing health care and human services in North Minneapolis for over 50 years. Position: The Youth Services Case Manager collaborates with black youth ages 12 to 24 to develop individual plans for school success, life skills, physical health and developing purpose. Successful Candidate Traits and Skills: Strong affinity for the mission, vision and values of the organization. Organizational skills with the ability to coordinate with others to drive effective youth service programming efforts and outcomes. Creative and innovative thinker that can identify and implement new ideas and solutions to engage and excite the guest and their families about the programs and services offered. A youth advocate that believes there is a need to address and support a continuum of social and health services that offer whole person, integrated services and results in the reduction of social, economic, and health inequities. A champion of diversity, inclusion and embraces cultural differences while honoring everyone's dignity, though process and contributions. A clear communicator who builds and earns trust by staying curious through open ended questions, listens with the intent to understand and uses a strengths based approach to identify and resolve issues. Understands how to lead in an unselfish manner which means it's not about you. It's about the people who make it all work. Qualifications: Associate degree in human services or related field with three years' experience working with a human services agency or organization in case management or care coordination, required. Bachelor's degree in human services or related degree, preferred. Experience working with justice-involved youth including a depth of knowledge on the impact of trauma and the ability to facilitate group empowerment groups and activities. Minimum two (2) years of case management experience, or experience delivering youth development programming. Minimum one (1) year working with youth and young adults from various ethnic and cultural backgrounds, ages, as well as economic circumstances. Ability to develop and maintain rapport with diverse groups of youth that encourages empowerment, self-sufficiency, and non-dependence. Experience identifying and assessing a wide range of children's needs and working with local community resources. Must have a valid driver's license, reliable transportation and the ability to meet NorthPoint's driving standards. Must be proficient in MS Word, MS Excel, and computer client tracking databases.
