Fairview Health Services Vad Coordinator in Minneapolis, Minnesota

Overview

The person performing this role is responsible for the coordination of inpatient and outpatient care for patients with a ventricular assist device. This coordination includes but is not limited to patient education, staff education, surgical coordination, clinic visits, and record keeping.

This position will work casual hours, Monday through Friday, typical business hours.

Responsibilities Job Description

Patient Evaluation/Care

  • Participate in heart transplant conference

  • Complete clinical evaluation of potential candidates for ventricular assist device intervention.

  • Assist with pump preparation and start up activities in operating room as needed by surgical team.

  • Interview and treat patients during clinic appointments within scope of practice.

  • Coordinate readmissions for patients having difficulties that require inpatient management.

  • Provide clinic support to patients leaving the unit for tests and procedures (i.e. operating rooms, heart cath laboratory, studies lasting over 90 minutes)

  • Assist with maintenance of adequate supplies for patients on device and for potential implants.

Education/Support, Patient and Family

  • Provide preoperative and post operative education to patient and family in collaboration with nursing staff

  • Develop and utilize patient educational guidelines for each device

  • Review and update guidelines annually and prn

  • Verify patient knowledge base prior to discharge from hospital

  • Document education provided, communicate with nursing staff about additional teaching required.

  • Serve as resource person to patients after discharge for clinical and technical support (on call 24 hours/day)

Education/Support, Health Care Professionals

  • Provide support and education to nursing staff, other health professionals and community health care professionals (home health nurse, referring physicians, first responders) who will have contact with the patient.

  • Educate on how to troubleshoot alerts and alarms. Come to hospital to assist with troubleshooting as needed.

  • Actively participate in local and national meetings to exchange knowledge and information in the area of cardiac surgery including cardiac assist devices.

  • Develop and integrate new and existing practices into the VAD Program Policy and Procedure Manual.

Qualifications

Minimum:

  • Baccalaureate degree in nursing

  • Current internal employees with 2 year Associate's degree in nursing may also be considered

  • Registered Nurse licensed to practice in the State of Minnesota

  • Minimum of three to five years’ experience in cardiovascular nursing, cardiac surgical nursing, critical care nursing, cardiac transplantation, and/or care of patients with ventricular assist devices

Preferred:

  • Master’s degree in nursing
