The case manager is responsible for assessing clients' needs, developing individual service plans, coordinating services, monitoring progress, and advocating on behalf of clients to ensure access to appropriate resources and services. The scope of the work and the requirements for supervision of the work is found in MN. Statute and in the MN. Health Care Provider Manual published by the Dept. of Human Services. Working Environment Hybrid Position: This hybrid position involves frequent travel to clients' homes and community settings, as well as working both in the clinic and remotely. The ideal candidate will be flexible, responsive, and maintain productivity and performance standards while working in various settings. Working hours for this position fall within standard clinic hours Monday - Friday. Key Responsibilities Provide adult or child case management services: 85% Assessment and Planning: Conduct functional assessments to determine clients' functioning level, needs, strengths, and preferences. Build relationships and trust to support goal achievement. Develop and implement individualized service plans with clients, families, and service providers. Identify and prioritize services to meet clients' goals. Empower clients to make informed decisions and take an active role in their care. Coordination and Referral: Facilitate access to services like mental health treatment, housing, medical care, and social services. Maintain collaborative relationships with community resources and service providers. Refer clients to appropriate services based on their needs. Assist clients in navigating healthcare, housing, social services, and education systems. Monitoring and Evaluation: Monitor progress toward goals as outlined in service plans. Regularly update service plans to reflect changes in needs. Conduct follow-ups to ensure effectiveness and utilization of services. Collaboration and Consultation: Work closely with multidisciplinary teams, including primary care and dental clinics, mental health professionals including psychiatry providers and therapists, and community and governmental social service agencies. Participate in case consultations, team meetings, and service planning sessions. Provide information, education, and support to clients and their families regarding mental health and related services. Crisis Plan: Develop and implement crisis plans to prevent and manage future crises. Coordinate with mental health providers and crisis mobile response as needed. Documentation and Compliance: Keep accurate, up-to-date records of interactions, assessments, service plans, and progress notes, completed within 48 hours. Participate in audits and quality assurance activities. Participate in team meetings and individual supervision (15%) Team meetings: Actively participate in team meetings, including morning huddles, adult or child team meetings, interdisciplinary case consultations, training sessions. Supervision: Engage in weekly individual supervision with your primary supervisor for case reviews and performance management. Qualifications Required Qualifications Training/Experience: Candidates must meet one of the following criteria: A. Bachelor's Degree: A BA/BS in behavioral sciences or a related field (e.g., social work, psychology, nursing) from an accredited institution, plus: At least two years (2,000 hours) of supervised experience providing services to individuals with mental illness, OR Fluency in a non-English language spoken by the ethnic groups served by CUHCC. B. Extensive Experience: At least 6,00 hours of supervised experience delivering services to individuals with mental illness. C. Graduate Student: Currently enrolled in a graduate program in behavioral sciences or a related field, with formal clinical training assigned by an accredited institution. D. Master's Degree: Holds a master's or other graduate degree in behavioral sciences or a related field from an accredited institution, with less than 4,000 hours of post-master's experience in treating mental illness--A combination of related education and work experience to equal at least 6 years. Child Practitioners: Must have specific training in working with children. Adult Practitioners: Must have specific training in working with adults. Licensure Requirements: Licensure is required only for candidates with a Social Work degree (BA, BS, or MA). Those candidates must have a Social Work license (LSW, LGSW, LISW, or LICSW) as per the Minnesota Board of Social Work regulations. Requirements: Valid driver's license and reliable transportation. Background check clearance as required by agency and state regulations. Qualification: Excellent communication and interpersonal skills. Strong ability to collaborate with clients, families, and service providers. Knowledge of community resources and mental health services. Demonstrates strong organizational and time management skills, with the ability to effectively manage a caseload, prioritize tasks, and plan efficiently Ability to utilize and complete all documentation processes in an electronic billing and health system in a timely manner. Respects and works well with a diversity of patients and staff. Experience handling difficult clinical situation, Knowledge or experience in de-escalating crisis situations. Preferred Qualifications: Experience working with clients with mental illness. Experience working in a community mental health clinic. Proficient in one of CUHCC's dominant languages: Vietnamese, Lao, Spanish, Hmong or Somali. About the Department CUHCC serves nearly 11,000 patients a year through over 55,000 visits annually.
