Minnesota Visiting Nurse Agency Staff Nurse RN (0.9 FTE) - Senior Care / Extended Care Clinics in Minneapolis, Minnesota
_SUMMARY_
Hennepin Healthcare is hiring aStaff Nurse RN(0.9 FTE: 4-5 days/week, 8 hour shifts) for our Senior/Extended Care Clinics in Minneapolis to work day shift (Mon-Fri).
A sign-on bonus is available for eligible external candidates (current employees and former employees returning to HHS within one year are ineligible).
_RESPONSIBILITIES_
Within the context of the Hennepin Healthcare System Relationship-Based Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by:
- Protecting, promoting wellness and optimizing health
- Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care
- Alleviating suffering through the treatment of the human response
- Partnering with the patient, their family and significant others
- Documenting interventions and an individualized plan of care
- Educating patient on their disease process
- Assuring continuity of care
- Educating patient and family in preventative care which promotes health and well being
QUALIFICATIONS
Minimum Qualifications:
- Graduation from an accredited school of nursing
- Possession of a valid license as a Registered Nurse issued by the State of Minnesota
Preferred Qualifications:
- Certification in area of clinical specialty
- Two years of ambulatory experience
- Prior experience working in Long Term care
- BSN Preferred. Bachelor of Nursing degree is strongly encouraged. Advancement in academic education will be discussed as part of individual professional development during the annual performance review.
