Minnesota Visiting Nurse Agency Pharmacist (Casual) – Specialty Pharmacy Services in Minneapolis, Minnesota

_SUMMARY:_

We are currently seeking a Pharmacist to join our Specialty Pharmacy Services team. This position is casual, no benefits, day/evening shifts, 8-hour shifts. Finalist will be required to pass Drug Testing.

/This position will be located in the Hennepin Healthcare Specialty Services Pharmacy. The individual will be responsible for pharmacy functions such as checking and verifying prescriptions, evaluating for drug interactions, and assessing medication profiles. The pharmacist will also be responsible for advanced functions such as accessing and interpreting lab results, documentation of outreach calls in the EHR (electronic health record), evaluation and documentation of adherence issues, and communication with specialty clinic providers and staff. The position will demonstrate skill with navigating an EHR for patient information, EHR documentation, specialty pharmacy accreditation standards, specialty pharmacy processes, and forming and maintaining relationships with clinic staff./

*Purpose of this position: *Responsible for the filling and dispensing of specialty and non-specialty medication orders including the proper handling, usage, and storage of drugs in the pharmacy. Maintains standards set by the department and third-party accrediting organizations and translates policies and procedures into the daily operation of the pharmacy. Promotes safe handling, usage, and adherence to specialty medications. Responsible for acting as an information resource to patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related information and literature. Oversees timely and accurate delivery of specialty medications. Supports the values of the organization by demonstrating personal responsibility, respect for self and others, innovation through teamwork, dedication to caring, and excellence in customer service.

_RESPONSIBILITIES:_

  • Verifies, prepares, compounds, and accurately dispenses medications
  • Responsible for interpretation of physician medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies and, based on an evaluation of the patient's condition, suggests appropriate or alternate drug therapy to the physician
  • Responsible for maintaining accurate and proper patient medication profiles, monitoring to ensure the proper choice of drug, dosage form, strength, length of therapy and interaction potential of the prescribed drug
  • Responsible for the dispensing, control, and record keeping of all controlled substances
  • Responsible for maintaining a working knowledge of and conforming to legal requirements, department policies and procedures concerning the distribution of specialty drugs, as well as good working knowledge of recent developments in the field of pharmacy
  • Assumes "charge pharmacist" responsibilities as required, for the coordination of pharmacists and supportive personnel, assigning work and delegating tasks and responsibilities, and for handling problems that arise during the absence or unavailability of the manager
  • Supports and assists the pharmacist supervisors, managers, and directors to integrate departmental and organizational goals into department’s activities
  • Assists in supervising and training pharmacy technicians, pharmacy interns, and support team members
  • May precept Fellows, Residents, or PharmD student rotations
  • Performs initial and recurring patient evaluation as it pertains to prescribed therapies. Collect, analyze, interpret, evaluate, and document appropriate data relative to medication therapy
  • Communicates and collaborates with medical staff to assist with specialty and non-specialty medication-related cost savings, clinical decision making, adherence interventions, and other aspects of medication therapy delivery as appropriate
  • Documents in the EMR or other database applicable patient interactions according to Specialty Pharmacy policy and procedures
  • Completes intake process for Specialty Pharmacy patients including new patient enrollment, insurance verification, and medication refill and delivery schedule
  • Monitors specialty medication adherence and proactively addresses adherence concerns and barriers
  • Complies with policy and procedures of pharmacy processes as it relates to third-party accreditation standards for specialty pharmacy
  • Completes other responsibilities as assigned including but not limited to: data collection, reporting, process improvement and maintenance of patient education materials

QUALIFICATIONS:

/Minimum Qualifications:/

  • Graduation from college with a degree in Pharmacy

/Preferred Qualifications:/

  • Advanced degree or enrollment in advanced degree program
  • Minimum of one year experience as a registered pharmacist working with specialty medications in a hospital setting
  • ASHP accredited residency in Specialty Pharmacy or related training
  • Board certification or advanced credentials in area of practice
