Fairview Health Services Nurse Coordinator in Minneapolis, Minnesota

Ebenezer offers a robust compensation package with excellent benefits and wages ranging from $29.55 - $41.00 / hour.

Ebenezer is hiring a Nursing Coordinator whom would be responsible for continuity of nursing care by monitoring nursing care delivered. This role demonstrates management skills through effective problem solving and decision- making, assuring compliance with State and Federal regulations as well as Ebenezer quality standards. This position assists with facility specific quality measures/indicators upholding to high quality standards and looking for ways to improve each measure through PIPP’s and other projects under the direction of the DON.

Nursing schedule includes;

  • 80 hours every two weeks

  • Day shift

  • No weekends

  • Leads assigned units and/or floors to assure that care is provided per individualized plan of care. Assures staff compliance with facility regulations and standards

  • Assures that plan of care, medical records and other documentations are up to date

  • Reports problematic areas to management and provides suggestions and solutions. Makes decisions regarding situations, implements actions and evaluates outcomes

  • Other duties as assigned and directed: Participate in meetings as requested, provides support to CNA’s for duties such as feeding, walking, ADL’s, and lift transfers.

Required:

  • Current MN LPN license or by start date

  • BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice

  • Client facing and internal communication skills

  • MS Office, Excel, and Word

Prefered:

  • Current MN RN license

  • 1-3 years Long Term Care Experience

  • Knowledge of project management

  • Point Click Care Applications (EHR)
