Lifesprk Clinical Support Specialist - Providence Place Senior Living in Minneapolis, Minnesota

Description Providence Place, a Lifespark Skilled Nursing Community, is looking for a dedicated and passionate Clinical Support Specialist to join our growing team. Providence Place is conveniently located on the bus route, located in southern Minneapolis in the Standish Ericcson neighborhood. Here we provide services for our seniors in a TCU (Transitional Care Unit) Care, Long-Term Care and Memory Care. We have been helping seniors Age Magnificently for over 25 years and are part of Senior Care Communities, a local non-profit supporting seniors. The position is responsible and accountable for mentoring/teaching nursing staff the knowledge and skill necessary to administer exemplary care, providing supervision, consultation, and assistance to nursing staff/other disciplines in the assessment, planning, implementation, and evaluation of care. Promotes excellence in customer service. Responsibilities: * Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation2. Management and supervisory experience in long term care. * Skills in planning, organizing, directing, communicating, motivating, and problem solving. * Has good leadership skills * Practice reflects current standards and follows facility policies and procedures. * Visits, monitors, and follows up on all new admissions. * Assures the NAR Team sheet accurate. * Analyze and recommend changes related to resident audits. * Investigates incidents and complaints and takes appropriate action in consultation with the Assistant Director of Nursing. * Prepares monthly reports such as but not limited to incidents, audits, and infections in a timely manner. * Checks resident "plan of care" for updates, significant changes, and short-term problems. * Supports and encourages educational programs and training of staff. Qualifications: * AA or BA/BS- in health care, social service, business * Minimum of 3 years related experience in health care education, program development, or healthcare leadership. * Experience in a health care setting is preferred.
