The Opportunity

The Intake Worker is a member of the multi-disciplinary team responsible for conducting initial assessments of incoming referrals, utilizing approved screening and assessment tools in the EHR, and determining treatment recommendations.

The ideal Intake Worker will foster community relationships and ensure timely access to care for individuals seeking services from our outpatient clinic. This role requires a commitment to FCA's mission to protect and strengthen Long Island's most vulnerable families, children, seniors, individuals, and communities through their work, and inspire others to do the same.

Responsibilities

  • Conduct initial screening and assessment of incoming referrals,and determine treatment recommendations.

  • Assist with linkage to health screenings, medication assisted treatment (MAT), and psychiatric assessments through communication with Care Coordination team.

  • Collaborate with external service providers to ensure coordination of care, and make referrals to outside providers as necessary.

  • Responsible for explaining treatment process and recommendations to participants.

  • Communicate with multi-disciplinary team and referral sources on assessment outcomes.

  • Function as an integral member of the treatment team and participate in case conferences, team meetings, and in-service workshops as scheduled.

  • Additional responsibilities as assigned.

Qualifications

  • High School diploma required, Bachelor's Degree preferred.

  • LCSW, LMSW, LMFT, LMHC, or CASAC required, candidates eligible for limited permit or CASAC-T will be considered.

  • Minimum of 1 year experience providing screening and assessment to substance use or mental health population.

  • Excellent verbal and written communication skills.

  • Must have intermediate computer skills with Microsoft Office.

Additional Information

Salary:$25.27/Hour

Schedule: Full-Time, 35 hours per week, this is a non-exempt position
