The Opportunity
FCA is excited to announce an opportunity for the position of Case Manager. As we continue to grow and evolve our Families Healing Together program, we are seeking a passionate individual to provide outreach and community education and support serving youth and families!
Responsibilities
Conduct targeted outreach to Suffolk County providers, schools, and other community resources to promote the program and services.
Collaborate and communicate regularly with external referral sources and service providers to ensure coordination of care.
Assist in the coordination of services and referrals of youth and families.
Provide community education on Adverse Childhood Experience (ACES).
Qualifications
Required:
H.S Diploma or Equivalent
Excellent verbal and written communication skills
Valid and clean New York State Driver's License
Preferred:
- 6 months related experience

Additional Information
Hourly: $26.37
Schedule: (M-F), evening hours, as needed
This role is full-time (35 hours/week), nonexempt
