Evidence Based Health Promotion Coordinator Department of Health and Human Services (DHHS) Pay Range: \$59,696 - \$69,888 Job Summary: The Evidence Based Health Promotion (EBHP) Coordinator is responsible for managing the Older Americans Act Title IIID evidence-based health promotion and disease prevention programming and services to advance the health and well-being of older adults in alignment with the County\'s vision and federal requirements. This position will develop community partnerships and resources to support evidence-based chronic disease prevention, health promotion, and evidence-based wellness programming. The EBHP will also facilitate training of community EBHP program facilitators, promote participation in EBHP programs by older adults, and ensure access as well as availability of EBHP programming to advance racial and health equity.
