We are well trained in both common and complex illnesses and have a particular focus on the care of patients with multiple chronic medical conditions. The Medical College of Wisconsin\'s Department of Medicine - Division of General Internal Medicine is seeking a Nurse Practitioner or Physician Assistant to provide a broad range of diagnostic and therapeutic services for the patients of the Primary Care Outpatient Services located in Milwaukee, Wisconsin at Froedtert Hospital Specialty Clinics. Our providers help patients understand disease prevention and the treatment of conditions including cardiovascular disease, endocrine disorders, cancer, and musculoskeletal disease. We also support patients with chronic diseases by partnering with other specialists to coordinate care. By working with our nursing staff, social workers, and community health workers we can deliver comprehensive care at an elevated level. The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching. The Ideal Candidate will be an experienced Primary Care APP who is looking to be a part of a fast-paced clinic and who wants to be part of a thriving and very collaborative academic practice at the Medical College of Wisconsin (MCW). Areas of responsibility will include, but not be limited to providing direct patient care to an adult population in an outpatient setting. In this role you will: Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring general medicine intervention in an outpatient environment as a member of the Internal Medicine Clinic Health Care Team. The team is responsible for providing management of primary care health and wellness, urgent care assessments and the treatment of co-morbid conditions such as but not limited to: diabetes, hypertension, heart failure, asthma, and anemia in an adult population. Writes accurate and timely progress notes, procedure notes, and/or admission and transfer orders.
