Bridges Healthcare Case Manager - CSS in Milford, Connecticut
Bridges Healthcare, Inc. is seeking a licensed Psychiatric LPN or RN
$50.00 per hour
5 hours per-week, non-benefited position.
PURPOSE and POSITION OVERVIEW
Provide psychiatric nursing services to adults and children clients of Bridges.
DUTIES and RESPONSIBILITES
- Serve as a resource to staff and clients
- Maintain and manage a medication injection clinic
- Serve as a liaison with home health care agencies that serve our clients
- Provide administrative support and follow-up to nursing supervisor and medical director
- Provide individual client education re: prescribed medications and general health education
- Maintain documentation
- Perform any other department or agency-related duties or special projects as directed by supervisor
QUALIFICATIONS and COMPETENCIES
CT State RN or LPN license required
Good communication skills and documentation and computer literacy/skills.
Knowledge of psychiatric illness and substance abuse.
Experience in counseling/therapeutic methods and practices and prescribing medications.
Ability to prioritize and organize schedules/demands.
Bridges is
