The University of Miami/UHealth Department of Otolaryngology is currently seeking a part time Social Worker to work in Miami, FL. The Social Worker provides psychosocial assessments, intervention, and/or treatment and discharge planning to patients and families to assist them in coping with their hospitalization, illness, diagnosis, and/or life situation and meet the continuing needs of patients post hospitalization.

CORE JOB FUNCTIONS

  • Interviews clients and their families and coordinates programs and activities to meet their social and emotional needs.

  • Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle.

  • Provides psychosocial assessments of patients and families to identify emotional, social, and environmental problems.

  • Formulates and implements plans utilizing appropriate social work interventions.

  • Collaborates with the healthcare team and involves the patients and families in the development and implementation of plans.

  • Assists with the screening, identification, and management of victims of abuse, neglect, domestic violence, and rape.

  • Makes referrals and serves as a resource for the healthcare team and serves as a patient advocate.

  • Maintains a working knowledge of relevant medical and legal issues and preserves records and statistics.

  • Attends and participates in staff, committee, department, and other administrative meetings.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

DEPARTMENT ADDENDUM

Department Specific Functions

  • Assist families in obtaining financial aid to support hearing loss services and/or obtaining devices.

  • Work collaboratively with program directors to manage and coordinate the hearing aid and cochlear implant loaner bank.

  • Collaborate with the health care team and involve the patient/family in the development and implementation of plans.

  • Facilitate case management across a multidisciplinary care continuum which includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.

  • Provide case management and referrals on an as-needed base.

  • Ensure there are no barriers limiting the patient from attending his/her appointments at the University of Miami Ear Institute.

  • Partner on school related issues with the Deaf Educator to provide family support if needed.

  • Assist with early intervention referrals and consistent communication with the Florida Department of Health Early Steps program.

  • Provide short term counseling to families and individuals that require emotional support during the cochlear implant evaluation process as well as post implant issues that arise.

  • Provide referrals for long term counseling.

  • Provide phone/face to face intakes to any patients and families to facilitate the start of hearing loss services.

  • Partner with Directors of CHP and Family Support Services to plan, coordinate, and attend CHP program events.

  • May function as a resource to other staff as appropriate.

  • Other duties as assigned.
