Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Responsibilities The Home Based Primary Care (HBPC) Clinical Pharmacy Specialist (CPS) must be a licensed pharmacist, preferably with a Doctor of Pharmacy degree, Clinical Pharmacy Practice Residency, or equivalent experience. The HBPC CPS initiates, modifies, and discontinues medications and performs comprehensive medication management (CMM) services via multiple modalities. The CPS improves clinical outcomes through the provision of CMM and applies principles of team-based care and population management to improve access, quality, and safety. The HBPC CPS is responsible for providing clinical pharmacy services and managing all aspects of the HBPC Program at the Miami VA Medical Center. Other primary duties of this position include but are not limited to: Clinical: Initial comprehensive medication assessment (CMA) documented in the electronic medical record within 30 days upon patient enrollment into HBPC, then quarterly (within 90 days) thereafter, and when clinically indicated. (a) The CMA will include but is not limited to a review of drug interactions, adverse effects, efficacy, appropriate indication for medications, disease state management, medication monitoring, medication adherence, and formulary conversion. Medication reconciliation may also be performed during the CMA or IDT meeting to ensure accuracy of medications. (b) Patient care visits include but are not limited to telephone visits, home visits, VA video connect, telehealth visits, MyHealtheVet, and secure messaging. (c) When appropriate in performing CMM, the CPS may make changes under their SOP as part of the initial and quarterly review. Participate in home visits, when clinically indicated and agreed upon, for comprehensive medication or disease state management (CMM) and/or medication safety concerns. Reinforce and provide education to the Veteran and caregiver(s) regarding proper use of prescribed and Over the Counter (OTC) medications in collaboration with other HBPC team members. Education may include proper medication storage, administration, dosing, indication for use, side effects, disposal, expiration dating, etc. Assist with coordination of care, such as communication with HBPC team members, other health care departments, outside health care providers, and patients/caregivers. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Perform informal therapeutic consultation ("curbside" consults) to provide guidance to team members on topics such as optimal medication use, drug information questions, formulary advice, as well as pharmacy/medication education, and reference materials for the HBPC team. Drug Information: Serves as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians and students. Uses appropriate references to research drug information. Has a good working knowledge of information resources, both written and electronic. Maintains a current knowledge of therapeutics and disease management. Provides timely and accurate responses to drug information inquiries from all customers. Education and Research: Pharmacy trainees (such as newly hired pharmacists, pharmacy residents, student pharmacists, Veterans Affairs Learning Opportunity Residency (VALOR) students, etc.) may be assigned to the HBPC teams of their individual precepting CPS. Specific assignments and oversight of trainees assigned to the HBPC CPS will be conducted under the oversight and guidance of the CPS. This may include creating schedules, competencies, coordination of experiences/home visits, training, mentoring, projects, presentations, evaluations, etc. The HBPC CPS may also provide education to the Veteran and caregivers on the proper use of prescribed and over the counter (OTC) medications, and medical devices, in collaboration with other HBPC team members. Education may include, but is not limited to, proper medication storage, administration, dosing, indication for use, adverse effects, disposal, and expiration dates. Able to adjust communication and tracking methods based on age and developmental considerations. Is able to accurately assess and document patient comprehension. Is able to refer patients to educational programs provided at the medical center by multidisciplinary teams. Skilled at using both written and verbal communication techniques in teaching others. Assists in new employee orientation sessions as scheduled. Education 1. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. 2. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. Must have 1 year of experience equivalent to the next lower grade level. Knowledge, Skills and Abilities. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Assignments. Candidates at this grade level are assigned as a Clinical Pharmacy Specialist. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
