JOB REQUIREMENTS: Job description Member is seeking dedicated personal care worker to work in his waterside home Are you a caregiver with a big heart? Do you enjoy helping others? Do you need to supplement your income by working a few hours at a time? Schedule: Friday, Saturday, Sunday and every other Tuesday evening The weekend schedule will be a combination of day shift 8:00am-1:00pm or evening shift 6:00pm-9:00pm *the shift times can be somewhat flexible to fit your schedule* Tasks: Showering/Dressing Meal Preparation Laundry/Linen Changing Medication Administration Transfers Occasional transportation to and from appointments or activities with the use of the member owned van Wage: \$17.00 per hour Ideal candidate must be reliable, a good communicator and have a caregiving heart There is a pet in the home and member is a smoker CNA/PCW workers encouraged to apply A driver\'s license is required for this position Who is Community Horizons: Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin.
