Job Title: PharmacistJob Description
The Pharmacist will be working in the Home Pharmacy that supports both adult and pediatric hospitals. The role involves providing home infusion pharmacy services, enteral therapy, and other specialized injectable therapies for pediatric patients and adults with childhood diseases such as cystic fibrosis and hemophilia. The main goal is to fulfill patients' needs before they are discharged from the hospital to ensure they receive proper treatment while at home. This includes working closely with chemotherapy, hydration antibiotics, and medication to help treat patients.
Responsibilities
Fulfill patients' needs before discharge to ensure they receive proper treatment at home.
Work closely with chemotherapy, hydration antibiotics, and medication to treat patients.
Clean up the database and go through the provider registry and built templates.
Provide coverage for existing staff, including doing rounds in the morning with nurses.
Conduct patient follow-ups and manage assigned specific shifts for the day.
Set up refills and make phone calls to patients.
Work with intake, delivery, and the IV room.
Essential Skills
High understanding of medications and their treatments.
Strong attention to detail and analytical skills.
Customer service skills.
Certified Pharmacy Technician (Cpht) certification.
Additional Skills & Qualifications
Pharmacist License.
3-5 years of experience, preferably in inpatient or home infusion settings.
Work Environment
The Home Pharmacy is offsite from the main hospitals in Menlo Park. Onsite, the billing team, intake team, fulfillment team, and delivery team are all in one location. The team has a great culture with a strong bond, being a smaller sized but very close-knit team. The shift is Monday to Friday with varying morning hours.
Pay and Benefits
The pay range for this position is $70.00 - $75.00/hr.
