What you'll need to qualify:

  • Current BLS; ACLS for critical care.

  • Willingness to float throughout the hospital and/or throughout multiple hospital locations.

  • EPIC experience preferred.

  • Cannot hold a concurrent position at a Trinity Health facility.

  • Ability to complete the orientation and training. We have different options to choose from that will meet your schedule.

ESSENTIAL FUNCTIONS

Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership, behaviors, practices and decisions.

Assesses, plans, evaluates, and coordinates management of patient care including discharge planning and utilization needs. Supports ministry and system goals for capacity management through achieving length of stay goals, identification of avoidable delays at department approved levels, and other Case Management initiatives.

Coordinates with the interprofessional team in meeting patient care goals.

Establishes partnerships with site Patient Experience champions to align efforts with patient satisfaction outcomes.

Identifies discharge needs, determines the appropriate level of care that the patient can transition to, coordinates planning, and problem solves continuity issues to ensure seamless continuum of patient care and prevent the patient from having to readmit to the hospital.

Ensures all appropriate clinical information is provided to outside agencies.

Communicates pertinent information to the appropriate members of the patient care team both effectively and professionally.

Identifies nd eliminates barriers to effective patient stay management and promotes improvements in work processes within department, hospital, and community.

Pursues professional development opportunities and maintains licensure.

Advocates for patient legal, social, and ethical rights while balancing patient resources and hospital financial stewardship.

Refers quality issues and poor utilization of services to the appropriate sources.

Coordinates/facilitates patient/family education.

Educates hospital leadership and colleagues to current case management practices, patient coordination issues, and unit specific trends.

Provides consultation to professional colleagues and employees regarding patient stay options, financial considerations, psychosocial concerns, risk management, and ethical issues.

Documents pertinent assessments, interventions, transitions, and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.

Utilizes computer systems.

Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

MINIMUM QUALIFICATIONS

Valid license as an RN. BSN Required Masters preferred.

BLS Certification Required.

3-5 years Case Management experience, hospital or community based.

A personal presence which is characterized by a sense of honesty, integrity, and caring with ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.

Must be comfortable operating in a collaborative, shared leadership environment.

Exceptional interpersonal and relationship building skills required to effectively collaborate with multiple stakeholders to obtain management, colleague, and physician involvement and accountability in achieving specific outcomes.

Ability to simultaneously address and prioritize multiple projects, demands and issues.

Must possess excellent verbal and written communication skills, presentation skills, advanced analytical skills, be well organized, and maintain a highly professional manner and appearance.

Demonstrated ability to operate in a collaborative, shared leadership environment.

Demonstrated high degree of self-motivation, assertiveness, and goal-attaining capabilities.

Effective negotiation, persuasive, business planning and verbal/written communication skills.

Ability to be flexible and adaptable to changing processes and schedule requirements.

Demonstrated ability to acknowledge and analyze formal and informal assessments of health care market trends, locally and across the country, and the impact/potential impact on the organization.

Possesses knowledge of budget processes, quality improvement, report distribution, The Joint Commission standards, state, and federal regulations, six sigma and LEAN methodologies.

Proficient in common software applications and electronic medical records. (Word, Excel, Power point).

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must be able to travel.

Must be able to adapt to frequently changing work priorities.

Must be able to set and organize own work priorities and adapt said priorities as they change frequently.

Manual dexterity is needed to operate a keyboard. Hearing is needed for extensive telephone and in person communication.

Pay Range: $42.00 - $47.00. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
