Centra Wellness Network Nurse Practitioner/Physician's Assistant in Manistee, Michigan

As a member of the Med Services Team staff is expected to provide support and coordination to all clients; to work in a collaborative effort and support systems development and change to improve services and care to persons with Severe Emotional Disabilities, Severe and Persistent Mental Illness, Co-occurring Illnesses, and Intellectual/Developmental Disabilities. CWN recognizes and provides best practice treatments and interventions that recognize all person's vulnerabilities to trauma, and traumatic experiences and that impact on daily living.

Medical Services team is comprised of Psychiatrist, Nurse Practitioners, Nurses, and Medical Assistants that ensure the development, planning, implementation and monitoring of high quality medical services and supports to children and adults. All services and medical treatment provided is under the direction of the Medical Director.

As a member of the CWN team staff will provide support throughout the agency, develop leadership skills, complete their tasks with efficiency and effectiveness and accept that all practices adhere to a recovery-based practice, providing the supports and guidance to empower, and promote recovery in clients and peers. Job Summary and Scope

Provides general medical care and treatment to consumers through evaluation, assessment, planning, education, intervention, and review in a variety of settings. Staff must possess computer, typing, grammar, and phone proficiencies; and have excellent communication and public relations skills to effectively interact with staff, customers, and community stakeholders. Additionally, the employee must be self-directed, demonstrate excellent critical thinking abilities, be able to exercise a high level of independent decision-making and work effectively in a team-based environment. The employee will complete other tasks as needed in various programs as assigned by Program Supervisor.

Position Specific -- Duties and Responsibilities

· Provides direct psychiatric services through comprehensive evaluation, diagnosis, treatment planning, and treatment of clients.

· Performs initial evaluations, psychiatric diagnoses, on-going assessments, and treatments of individuals with severe and chronic mental illness.

· Assesses, evaluates, and identifies consumers' needs and makes treatment plans and referrals in collaboration with existing medical staff and established Nurse Practitioner protocols.

· Uses the Agency's protocol for eligibility determination, medical necessity and therapeutic appropriateness when recommending levels of care, intervention, and support.

· Provides psychiatric medication and medication assisted treatment reviews and consultation services within appropriate boundaries.

· Serves as a liaison for the Agency with community physician, physician health homes, area hospitals, local medical societies, and other medical professionals and agencies.

· Communicates and initiates appropriate Agency referrals.

· Serves as a resource to staff in medical and clinical matters.

  • Leads the development of medical services and supports. Ensures implementation using established guidelines, resulting in high quality efficient and effective mental health services.
