Position Summary: **Under the general supervision of the Clinical Nurse Manager/Registered Nurse, and the Community Health Center Administrator and in accordance with established policies and procedures, the Licensed Practical Nurse assists medical providers in rendering patient care during clinic visits by performing routine technical tasks and other duties, as required or assigned.

*Essential Duties:* **

  • Responsible for collecting information from patients, i.e., medical records, medical insurance details and other pretreatment formalities, prior to patients being seen by a medical provider.

  • Responsible for triaging and gathering health details about the patients, such as checking vital signs which usually comprise blood pressure, body temperature, respiration and heart beat rate, etc.

  • Helps patients get ready for medical examinations; should the medical provider prescribe any medications, immunizations and/or injections, the LPN would be responsible for administering these to patient.

  • Should the patient have external wounds on his/her body, the LPN would dress the wounds for quick healing. After recording the present medical condition of the patient, the LPN is to inform the respective provider regarding same and report any pertinent changes in the patient’s condition or needs.

  • Responsible for patient’s health education; must inform the parent/patient about the patient’s regular medications, healthy diet and provide guidelines for maintaining his/her health.

  • Documents the care and pertinent client reactions at each visit and records same in progress notes according to Children’s Aid (CA) policies and procedures.

  • Assumes responsibility for personal growth by attending Agency staff meetings and in-service programs as required by the RN.

  • On a bi-monthly basis review vaccine inventory and order vaccines accordingly.

  • Routinely update CIR Registry, or as information becomes available.

  • On a monthly basis review medication inventory and provide a report to Nurse Clinical Manager.

  • Perform Visual Exams per protocol for children in foster care.

  • Provides a cooperative effort toward quality health care by understanding functions of other members of the health care team.

  • Accepts the philosophy, purpose and policies of Children’s Aid Health and Wellness Division

  • Maintains Universal Precaution Guidelines and safety measures according to CA policies and procedures.

  • Informs the primary RN or Nursing Supervisor if changes are noted in a patient’s chart.

  • Maintains client confidentiality.

  • Ensures that all patients, visitors and coworkers receive personalized prompt attention and are treated with dignity and respect.

  • Acts as a patient advocate.

  • Responsible for keeping the Clinical Nurse Manager/Registered Nurse, and the Community Health Center Administrator, or designee updated on current issues and/or problems related to your job responsibilities or the Center’s environment.

  • Participates in meetings, trainings, events, quality improvement and other initiatives

  • Comply with all privacy laws including HIPAA and other regulatory laws, rules and guidance.

  • Performs other related duties as assigned.

Minimum Qualifications:

  • Graduate of an accredited practical or vocational training nursing program.

  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the State of New York.

  • Two years nursing experience in a clinical area.

  • Minimum High School Diploma/GED required.

Key Competencies:

  • Continuing Education Unit Accreditation Maintenance.

  • Bilingual (Spanish/English) preferred, with ability to interpret both languages and act as an interpreter.

  • Works effectively and enthusiastically with children and adolescents, families, professionals and other personnel from a wide range of cultural, social and economic backgrounds.

  • Has excellent observation, oral and written communication skills.

  • Must be able to handle multiple tasks at one time.

  • Knowledge of Electronic Medical Records, preferably, EClinicalWorks.

  • Strong organizational, customer service, and interpersonal skills.
