POSITION SUMMARY:
Responsible for implementing systems to ensure that all required incident related documentation is completed and submitted within required time frames. Assists and provides technical assistance to all programs under the Auspice of Constructive Partnership Unlimited – Metro Services in managing and maintaining documentation required to be in compliance with applicable Incident Management regulations.
POSITION DUTIES: 1. Develops, implements and maintains a tracking system for all documentation required to be completed, obtained and maintained by the Quality Assurance and Quality Improvement Department, including but not limited to: Investigations and required attachments; Form OPWDD 147 and 148 for all Notable, Significant and Reportable Incidents. Jonathan’s Law notifications, Corrective Action Plans (CAP) and required attachments.2. Enters data into OPWDD’s Incident Reporting Management Application (IRMA) software, ensuring all applicable notifications are completed for the OPWDD 147 within 24 hours as required. Provides written notifications and letters to regulatory agencies, healthcare providers, advocates and families as required/requested. 3. Reviews all Notable, Significant and Reportable Incidents for completeness focusing on ensuring incident report has all required elements completed, thorough investigation, appropriate corrective actions and necessary attachments. Submits full investigative records of all Serious Notable and Reportable Incidents to OPWDD and the Justice Center, utilizing IRMA and the JC’s Web Submissions of Investigation Reports (WSIR).4. Attend the monthly Incident Review Committee meetings; develops and maintains meeting minutes for all open investigations/incidents on a monthly basis. Complete reporting updates within IRMA for open investigations.5. Participates in CAP meetings and/or communicate with Program Personnel to ensure team members are acquainted with the required steps to fulfill recommendations timely. Review Plan of Corrective Actions to ensure the disciplines taken satisfies the recommendations based on the investigative outcome. Upload required documentation within IRMA. Generates reports of outstanding recommendations for monthly distribution, and as needed. 6. Collects and maintains incident management data to identify trends, contributes data for the annual trend report by providing analysis of all incident management activities and statistics for every site7. Coordinates with Human Resource Department to complete and submit required forms related to Incident Management Specialist Job Descriptions: Quality Assurance and Quality Improvementcurrent and past employees involved in Reportable Abuse and Neglect Incidents. Obtain personnel information from the HR Dept. for current Reportable Abuse and Neglect investigations and develops written notices to persons identified. 8. Maintains and conducts periodic record review of incident management records internally and within IRMA to ensure all required documentation is present. 9. Complete daily review of IRMA and the agency’s dedicated mailbox for important communication related to incident management. Alert team members of urgent correspondences to ensure timely follow-up.10. Communicates with OPWDD’s Investigations, Incident Management and Corrective Action Plan Departments on an as needed basis. 11. Prioritizes job tasks to ensure timely and accurate completion of required work. 12. Performs all other duties as assigned.
REQUIREMENTS: 1. Proficient in Microsoft Office (Word, Excel, PowerPoint and Outlook) and Access Data Base systems.2. Associates degree or 60 college credits in human service field preferred.3. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
