Responsibilities include but not limited to:

· Help the rehab staff with patient interaction and coordinate treatment plans

· Complete clerical  work as directed for residents and  team member s

· Participate in department meetings and in facility meetings as assigned

· Answer and direct phone calls

· Maintain compliance and regulatory driven duties such as obtaining physician signatures, maintain quarterly screen compliance and any other State or CMS regulations

· Coordinate work activities with members of other departments and other staff within the rehabilitation department

· Coordinate therapy schedule with nursing if needed

· Assist patients, clients, team members with issues or needs

· Participate in facility performance improvement programs

· Report any unusual incidents to Administrator

· Assist in maintaining the cleanliness of the treatment areas and department

· Assist with maintaining an adequate stock of supplies and equipment  including pool check/maintenance

· Perform other duties as requested

EDUCATION

· High School graduate or equivalent.

EXPERIENCE

· Previous experience in patient/resident care in a nursing center, hospital, or rehabilitation center preferred.

· Certified Nursing Assistant (C.N.A.) preferred

· Therapy students will be considered.

•            730am or 8am in, semi flexible  schedule

•            Usually no weekends
