BASIC FUNCTION
- Conduct medical history taking, exam, diagnosis and treatment and complete EHR
- Interpretation of lab work.
- Perform routine outpatient procedures for which they have adequate training and experience.
- Supervise nursing staff.
- Counsel patient, patient education
- Assist in chart review.
- Assist in policy and protocol development.
- Other Duties as assigned
PRIMARY RESPONSIBILITIES AND DUTIES
- Provide comprehensive medical care to patients of all ages.
a. Conduct thorough patient assessments, including medical history, physical examinations, and ordering diagnostic tests as needed
b. Diagnose and treat acute and chronic illnesses, injuries, and conditions within the scope of family practice
c. Develop and implement personalized treatment plans, including medication management, lifestyle modifications, and referrals to specialists when necessary
- Establish and maintain patient relationships
a. Build strong patient-provider relationships based on trust, empathy, and effective communication
b. Educate patients about their health conditions, treatment options, and preventive measures
c. Address patient concerns, answer questions, and provide appropriate medical advice and follow-up care
- Perform medical procedures and minor surgeries as per training and credential privileges
a. Prescribe vaccinations, immunizations, and medications as appropriate
b. Perform minor surgical procedures, such as suturing, wound care, and lesion removal as approved by credentialing Chief Medical Officer.
c. Perform basic office-based procedures, including skin biopsies, joint injections, and minor orthopedic procedures as approved by Chief Medical Officer.
- Collaborate with interdisciplinary healthcare teams
a. Work closely with medical providers, medical assistants, and other healthcare professionals to coordinate patient care and ensure continuity
b. Participate in interdisciplinary team meetings to discuss patient cases, treatment plans, and follow-up care
c. Provide consultative services to other healthcare providers within the health center as needed
- Maintain accurate and timely medical records
a. Document patient encounters, medical histories, examination findings, diagnoses, treatment plans, and progress notes in electronic health records (EHR)
b. Ensure compliance with legal and regulatory requirements for medical recordkeeping, privacy, and confidentiality
c. Review and update medical records regularly to reflect changes in patient conditions and treatment plans
d. Timely completion and closure of charts
- Stay current with medical knowledge and best practices
a. Engage in continuous professional development through participation in conferences, seminars, and medical literature review
b. Keep abreast of advancements in family medicine, evidence-based practices, and clinical guidelines
c. Maintain board certification and necessary licensure, fulfilling continuing medical education (CME) requirements
- Contribute to quality improvement initiatives
a. Participate in quality improvement programs to enhance patient safety, satisfaction, and outcomes
b. Contribute to the development and implementation of clinical protocols and best practice guidelines within the family practice department
c. Monitor and evaluate clinical outcomes, identify areas for improvement, and collaborate with colleagues to implement necessary changes
- Engage in community outreach and education
a. Participate in health promotion and community education activities, such as health fairs, workshops, and preventive care campaigns
b. Serve as a representative of the health center in community events and initiatives
