JOB REQUIREMENTS: Respiratory Therapist Discharge Planner - 35700 DESCRIPTION/RESPONSIBILITIES: Work Schedule : 90 % FTE, 36 hours/week. 8 or 10 -hour day shifts scheduled between the hours of 6:15 AM - 1:45 PM . We are seeking a Respiratory Therapist Discharge Planner to: * Collaborate with the Nurse Case Manager and the entire care team in the development of care standards and guidelines for improving quality of care, collaborating discharge, and reducing re-admissions. * Offer clinical expertise in assessment, recommendations and implementation of care plans/equipment.
