Responsibilities:
Provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragement
Provide interventions in the office, virtually, in the community and in client’s residences based on client preference, client need, and at the clinical direction of the Team
Enroll eligible individuals in care management, including BHCP and Health Home
Collaborate with Care Transitions Team members to triage hospital admissions data and formulate assertive follow-up strategies to contact hospital treatment teams and enrollees during inpatient admissions
Assist the team and individuals in providing on-going assessment of symptoms and responses to staff interventions
P articipate in the development of treatment plans for each individual in which you are a member of their treatment team. Ensure that treatment goals are culturally competent, strength based, realistic, attainable and promote recovery.
Ensure that each individual is actively involved in their treatment planning process and their preferences are embraced
Ensure that documentation is written is measurable, observable, objective and utilizes clinical terms
Coordinate and ensure timely face to face contact with enrollees in the community following inpatient discharge within timeframes dictated by MassHealth contract
Qualifications:
Must have bachelor’s degree or master’s degree in psychology, social work or related field and three years experience
Must have a strong commitment to recovery oriented services
Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses, kids, families and respect for their preferences for treatment
Must have ability to communicate clearly and effectively with individuals in rehabilitation and treatment
Salary: $50,000
Hours: Monday-Friday, 9am-5pm with remote/hybrid flexibility(3 days on-site, 2 days remote)
Bilingual Differential: $3,000
