As a Clinical Community Health Worker (CHW), you will be responsible for assisting the UCLA Health team members and patients to access, navigate and maintain needed social care services both within and outside of UCLA Health. You will be a part of Ambulatory Care Management Department and work under the supervision of the clinical and operations management in collaboration with other CHWs and Clinical Advisors.

*This position is 25% flex-hybrid and includes significant field work (patient homes, community and office visits with patients) as well as administrative support to assist the team in bridging clinical and social care.

Additional responsibilities include, but are not limited to:

• assist in partnering with community based organizations, connects patients and their families to social drivers of health (SDOH) related services including but not limited to food, housing, transportation, employment, child care, education, and mental health services

• supporting patient social care needs assessments, advocating for patient preferences and resources, assisting in the identification of needs and resources, referral, outreach to relevant community partners, and assisting the team in designing and implementing educational efforts

• helping the team to understand and interpret data reports from a community member and patient's perspective

• facilitating patient's and caregiver's enrollment in programs housed within UCLA Health and/or externally

• engaging actively with local community based organizations, helping establish and maintain partnerships, supporting patient caregivers, and other key stakeholders while exercising culturally sensitive, culturally responsive, and appropriate strategies

Salary Range: $63,800 - $127,000 Annual

Qualifications

Required:

• Minimum 2 years of work and/or knowledge of the public health or social care needs in UCLA Health service areas.

• Verbal and written communication skills in English enough to be able to communicate to patients and the social care team members in person, virtually, and by telephone.

• Strong interpersonal skills with ability to connect with a diverse patient population, especially those who are highly vulnerable and/or non-English speaking communities

• Ability to use computer software and hardware such as Microsoft Word, Teams and Outlook.

• Skill in recognizing an effectively responding to emergency or high priority situations that need to be elevated to supervisor, Clinical Advisors, and/or patients.

• Ability to exercise diplomacy and flexibility in interacting with others.

• Understanding and ability to champion the development of community partnerships related to social drivers of health programs.

• Carry a cellphone during normal work hours and respond to urgent and emergency calls in a timely manner.

• Ability to problem solve with patients and/or in collaboration with other team members.

• Ability to analyze problems and formulate suggestions and solutions that advocate for and respect patient and caretaker preferences.

• Strongly flexible and able to maintain a professional and calm demeanor when plans and implementation tasks change.

• Ability to follow instructions and communicate important considerations to Clinical Advisors and management team.

• Ability to advocate for the patient and/or patient preferences within the internal team and with external partners.

• Demonstrate a favorable personality, respectful and cooperative attitude, maturity level, initiative, and enthusiasm; display professionalism, respectful and productive interpersonal relationships.

Preferred:

• Community Health Worker Certification - strongly preferred

• Ability to speak and write in a language other than English (e.g., Spanish, Korean, Mandarin, Tagalog, Armenian, American Sign Language, etc.) is strongly preferred

• Training and experience with diverse patient populations including but not limited to those who experience homelessness, low SES, limited English proficiency, and others underrepresented groups.

• High school diploma/GED
