Duration: 12+ months Hours/Week: 40.00 hr./week location: 1580 State Rt 56 London OH 43140 Note: CPR certification Ohio Medical License LISW or KPCC Roles and Responsibilities: Treatment team member, conducts advanced practice individual and group clinical interventions as prescribed on the individual mental health treatment plan, using clinical intervention strategies within their scope of practice and directed by the BHP Supervisor or Mental Health Administrator. Provides clinical case management services to an assigned caseload, including the monitoring of the offender's compliance with the mental health treatment plan, and completes biopsychosocial assessments and mental health status examinations as part of interdisciplinary treatment planning. Completes transfer and discharge summaries, and SSI requests; consults with other institution staff regarding the mental health caseload inmate's institutional adjustment and service needs. Initiates and maintains contact with families and community agencies as necessary. Seeks information relative to patient care and treatment pre-incarceration. Makes appropriate community and re-reentry referrals per mental health treatment team. Documentation of all incarcerated person encounters in the electronic healthcare record, discharge, release and wellness plan development. Completes miscellaneous forms and reports as needed per policy and protocol; attends meetings with supervision, attends in-service training sessions and workshops for professional development (CEUs); participates in peer reviews as assigned. Updates patient MH classifications and supportive functions. Required Education: Master Degree.
