Meridian Behavioral Health Cr Peer Specialist in Live Oak, Florida
- Work with mentally ill and/or substance abuse individuals at risk for incarceration or incarcerated. Incorporate motivational interviewing and recovery-oriented engagement strategies. Advise court evaluators and attorneys of appropriate treatment and placement alternatives for offenders.
- Coordinate all jail releases on clients and monitor. Use Re-Entry Checklist to assess needs, monitor discharge needs. Checkpoints will be (1) prior to release from jail, (2) during establishment of the treatment plan following release from jail, (3) three months following program admission, and (4) discharge. Maintain that all service requirements are implemented, monitored and reported to the courts in Alachua County. Share information and coordinate transition to later intercept points.
- Provide reentry services such as treatment, intensive case management, benefits coordination, vocational services, housing, and systemic collaboration. Maintain a central file for communication to and from agencies.
- Track when court reports are due and complete documents for court and attend hearings as scheduled.
- Monthly face to face and at least bi-weekly contact.
- Performs other duties as required to meet program and departmental goals and objectives.
